1. Offer Acceptance
By scheduling an appointment or accepting any offer from Your Family Dentist (from now on referred to as “the Clinic”), you agree to be bound by these terms and conditions.
2. Offer Details
- Free Consultation: Your Family Dentist offers complimentary dental implants, dentures, and veneers consultations.
- Dental Implants: Dental implant procedures may qualify for a discount of up to $300. The specific discount will be determined based on individual patient needs and treatment plans.
- Dental Veneers: Dental veneers start at $849. The final cost will depend on the number of veneers required and the specific materials used.
- Check-up and Clean-up: This service is available at a discounted rate of $149.
3. Offer Changes
Your Family Dentist reserves the right to modify, alter, or discontinue any offer at any time without prior notice.
4. Treatment Planning
All dental treatments at Your Family Dentist require a comprehensive examination and diagnosis. The final treatment plan and associated costs will be discussed with the patient before any procedures are performed.
5. Payment
Payment plans and financing options may be available. Please inquire about available payment options at the time of service with Your Family Dentist.
6. Insurance
Patients are responsible for verifying their insurance coverage and benefits before treatment. Your Family Dentist will assist patients in submitting insurance claims but cannot guarantee payment or coverage.
7. Appointment Cancellation
Patients are required to provide at least 1 hour’s notice for appointment cancellations with Your Family Dentist. Failure to cancel within the required timeframe may result in a cancellation fee.
8. Limitation of Liability
Your Family Dentist shall not be liable for any indirect, incidental, or consequential damages arising from the provision of dental services or the acceptance of any offer.
9. Governing Law
These terms and conditions shall be governed by and construed in accordance with the laws of Sydney, Australia.
